What we do

Four roles. One connector function the system keeps wishing it had.

JSPHI is not a clinic and not a competitor. We are the connection function the public health ecosystem already needs, made institutional.

Connect

Bring partners into relationship.

Map who already serves whom. Make the warm handoffs that keep people from disappearing between systems.

  • Asset and gap mapping across a place
  • Warm-handoff protocols between partners
  • Cross-organization referral pathways
  • Shared care navigation infrastructure
Amplify

Carry community voice into rooms that matter.

Community knowledge belongs at the same table as institutional knowledge. We make sure it shows up there.

  • Community-led research and storytelling
  • Policy-room briefings rooted in community testimony
  • Data interpretation that includes who is missing from it
  • Public reporting that names what communities are seeing
Translate

Cross the languages of care without losing meaning.

Community to clinic. Clinic to policy. Policy back to community. Translation is craft, not paraphrase.

  • Plain-language guides for insurance, referrals, and bills
  • Multilingual care navigation tools
  • Briefings and trainings for cross-sector partners
  • Communication coaching for community-facing programs
Convene

Host the rooms that did not exist.

Quarterly cross-sector tables where decisions move and community sits as a full participant, not an audience.

  • Quarterly Community of Care tables in each place
  • Cross-functional working groups on named gaps
  • Annual learning convenings across pilot sites
  • Co-design sessions for new shared services
Before you reach out

What JSPHI can help with, what is free, and where our role begins and ends.

Plain-language support for community members and partners deciding whether JSPHI is the right place to engage.

When to use JSPHI

If you serve people the system regularly misses.

FQHCs, CBOs, faith institutions, schools, and public health departments can engage us when patterns emerge that no single organization can solve alone. Community members can engage us at any time, in any place we have a Community of Care underway.

What is free

For community, always free.

Story collection, navigation support, plain-language guides, and pathway connections are free for community members. Partner organizations engaged in a pilot Community of Care receive convening, mapping, and translation services as part of the shared work.

What JSPHI is not

Not a clinic, a competitor, or a payer.

We do not provide medical care. We do not deliver direct social services. We do not replace existing partners. We do not lobby. We do not sell community data. We hold the connections between the parts.
