Share where care became hard to reach.

Your story is treated as evidence, not data. What you share helps JSPHI name patterns in rooms where decisions about funding, policy, and partnership get made. You decide how it gets used.

  • Share a story. Three sentences is enough. Anonymous if you want.
  • Add your voice to the Community Health Access Study, a survey about insurance, barriers, and what you have experienced.
  • Join a listening session. Held in community spaces across Delaware, Texas, Maryland, Arkansas and virtually beginning in 2026.
  • Help shape what JSPHI builds. Patterns from community stories drive what we work on next.

We never share what you tell us without permission. We do not need your full name, your diagnosis, or any clinical detail. Share only what you are comfortable sharing.

Tell us where care became hard to reach, understand, afford, or continue. What you share helps JSPHI name the pattern in rooms where decisions are made. You decide how your story may be used.

Please do not include clinical details you are not comfortable sharing.
