Fund the connective tissue.

JSPHI is seeking founding philanthropic partners across twelve, twenty-four, and sixty-month horizons. We are building the connector function that the public health ecosystem already needs, and that no single grantee can fund alone.

Most philanthropy funds parts of the care system. Few funders are positioned to fund what holds the parts together. JSPHI exists to be the connective tissue between the partners you are already funding, so the outcomes you are paying for survive after the grant cycle ends.

A standing team of community connectors, navigators, and convening leads. The Community Health Access Study and the listening infrastructure that drives it. Plain-language translation tools at scale. Pattern briefs and policy follow-through. A pilot Community of Care in Texas, with replication learning built in.

12 months

Build the core team. Launch the pilot Community of Care in Tarrant County. Field the second round of the study.

24 months

Stand up the convening infrastructure across five care areas. Publish the first regional pattern briefs. Add a second Texas community.

60 months

Replicate in two additional states. Stand up the policy and amplification function as a permanent program. Build durable funder coalitions.

Quarterly briefings on what we are hearing. Direct access to the Executive Director. Co-branded pattern briefs when appropriate. Invitations to convenings of partners and peer funders. A clear theory of change with metrics that match how connector work actually moves outcomes.
